Your question is reasonable. When new drugs hit the market the dose
used is usually higher than what is recommended down the road. This
is because the pharmaceutical companies want to impress the
physician and the patient with a show of immediate efficacy. So
often this means higher toxicity.

Other factors that may be considered include half-life (36 hours),
urgency of needed response, standard dosaging, your albumn levels,
whether you smoke (faster clearance), volume of distribution (232
liters), and conflicts with other meds (e.g. competing for specific
hepatic oxygenases). Most physicians just consider the usual dose
and check any warnings or contraindications.
